CMD REGISTRATION: (FORMS)

Prior to any participation in the CMD Program, prospective divers and parents must complete the following forms:

1) Diver Registration Form
2) CMD Diver Payment Plan
3) Authorization for Medical Treatment & Emergency Medical Information Form (Signed by parent)
4) Waiver and Release of Liability Form (Signed by parent and diver)


5) Also must have Holy Cross WAIVER which is not included with CMD forms.. 

***All divers must obtain a yearly membership in Amateur Athletic Union (AAU) which costs $16 (added benefits) ***

CMD TEAM REGISTRATION

CMD TEAM MONTHLY TUITION FEES

--->    There are responsibilities and expenses associated with a diver’s participation in the Central Mass Diving.  The primary expense are the program fees payable monthly to Wil Aybar or CMD. Fees are still paid if a diver is unable to attend due to injury or illness.

COACHES MEET FEE

 Away meets.

If the meets are around the MA & RI area and does not require a stay over night the fee will be $25 per Diver.


If a stay over is required then I will take the lodging price and divide by the numbers of divers attending the meet plus $25 per Diver.


If the meet is an AAU National Championship meet--> Usually in the warm climate (Fl, CA, etc). The divers will have to pay for the cost of total trip to event. CAR, FLIGHT, HOTEL and COACHES FEE. The total cost will be divided by the amount of divers attending the meet.


If the away meets are located in (ME,NH,CT,NY,NJ etc) Where a hotel stay will take place then the coaches fees will be paid by the team. The more CMD divers attend the cheaper the cost.


*****If meets are hosted at Holy Cross our Home then there will be no coaches fee all I ask is that you help run the table due to lack of staff. I can teach you how to add the divers scores etc…*****


****Reflecting the spirit and effort of our program, we ask that parents should try to set an example in the diving community with enthusiasm, cooperation and helpfulness.  We are all admirers of our sport

MEET ENTRIES
    It is the responsibility of each diver to submit his/her entry form and entry fee.  Meet fees varying from $30-40 per event. Meet information will be posted on the website and is distributed by the coaching staff.

~Important Survival Tips~

1)      Learn to fill out diving sheets early. It is your responsibility
2)      Don’t disturb coaches while they are coaching
3)      Parents not allowed on deck during practice or during meets unless you are helping run the scoring table which is recommended.
4)      Parents~ Don’t try to coach your diver. Support them, encourage them be their parent. They get enough corrective talk during practices.
5)      Divers are not allowed to coach other divers, unless approved by coach.
